Since elders’ needs can be wide ranging we are prepared to respond to the breadth and depth of those needs. This means assessing the situation to develop a person centered care plan that can be targeted to a specific, time-limited problem or a more comprehensive long-term approach. We provide improved access to elder care services in our community, with guidance on the best quality and value. We support elders as they navigate the health care system, advocating for optimal outcomes. We keep families who are at a distance or stressed with other responsibilities up to date. We give practical advice and emotional support to lessen the burden of life changing events. We are available as needed to our clients and their families and sometimes just knowing you are part of a team and not alone can make all of the difference.
